The southern terminus was moved from Shenandoah Heights to Frackville in 1946.

A median was installed in 1957 from Gilberton to Shenandoah.

In 1965, the route was widened and a median installed between Frackville to Gilberton and a year later from Humboldt to Hazleton.

Usually it is private citizens who fight for changes to a road, but calls for changes to this route came from an unlikely person.  After a deadly tractor trailer crash that claimed the life of the drive in March 2012, the Schuylkill County coroner reached out to PennDOT to get more warning devices for the sharp curve at the bottom of the hill in Brandonville.  There is only one sign to warn drivers about the conditions ahead.  Since 2000, there have been more than 20 crashes in the area of question.  "One of my themes of my coronership is keep the people out of the morgue," said Doctor David Moylan.  PennDOT had received a request to do a safety review on that stretch, and would be finished at the end of April 2012.
